  • Overview

    Our company, Elderwood is currently seeking a transformational leader to develop and expand nursing services for our Skilled Nursing Facilities on a part time basis in Rhode Island.

    The regional nurse consultant will be a registered nurse and will have demonstrated evidence and experience in the long term care and the post-acute space. They will be assigned to provide clinical oversight to skilled nursing facilities within their geographic location. The individual will have and express a genuine interest in caring for elderly patients and have experience in clinical operations.

    Candidates should be very detailed with reports specifying any opportunities for improvement from the audits and investigations. Additionally, they will provide recommendations for corrective actions and monitoring. Candidates should be self-starters and comfortable working with minimal supervision.

    The regional nurse consultant reports to the Chief Nursing Officer and have a dotted line to the regional director of clinical services.


    • Act as a nurse mentor and advisor to facility leadership and assist in Director of Nursing orientation, by being available for consultation, keeping current with medical and nursing care processes, keeping abreast of changes in health care delivery systems and methods and providing information to community leadership as indicated or requested.
    • Provide onsite support with annual state and federal survey inspections.
    • Knowledgeable of and assist in implementing state and federal regulations and the company’s policies and procedures.
    • Assist in developing a plan of correction following a state or federal survey in cooperation with facility leadership.
    • Assist, as needed or requested, in development of adequate staffing patterns and ratios in cooperation with facility leadership.
    • Notify timely the appropriate company leadership of events which pose potential compliance and/or litigation risk. Assist facility leadership in managing, investigating, or providing follow-up education related to such events.
    • Conduct mock survey audits and perform onsite review of resident care and services on a routine basis and as requested, to ensure compliance with state and federal regulations and company policies and procedures. Provide a verbal and/or written report of findings and recommendations for follow-up to the Administrator, Director of Nursing and appropriate company leadership. 
    • Maintain effective communication with facility leadership by reporting all observations while onsite to the Administrator and Director of Nursing. Conducting entrance and exit conferences with the Administrator and/or Director of Nursing or designees during each onsite visit.
    • Communicate routinely and effectively with the CNO, RDCS and RDO regarding facility clinical performance, site visit schedules, survey activity, events or other clinically relevant information.
    • Assist in nurse recruitment and retention efforts as needed or requested. Participate in the interview and selection process for community nursing leadership positions as needed or requested.
    • Observe and emphasize safety awareness and accident prevention to facility staff.
    • Review quality measures and quality data (CMS 5 Star, CASPER, QM/QI, Quintile Reports, etc.), assist facility leadership in development of performance improvement programs (PIPs), action plans and review outcomes in relation to established benchmarks.
    • Assist in developing and conducting ongoing training programs for nursing and other staff in response to QAPI programs to promote quality care and survey readiness at all times.
    • Attend and participate in facilitate QAPI Meetings and Risk Meetings. Review quality measures and quality data, and QAPI assist in development of organizational performance improvement programs, action plans, PIPs, and review outcomes in relation to established benchmarks. Notify the CNO, RDCS, and RDO of any concerns including failure to follow-up on recommendations.
    • Participate in the creation, revision and updating of policies and procedures in accordance with changes in standards of practice, state, and federal regulations.
    • Participate in healthcare associations and professional organizations.
    • Collaborate and coordinate with the company’s business partners including contracted consultants, vendors and other healthcare providers and partners.
    • Submit required reports to company leadership in an accurate and timely manner.
    • Pursue goals as established by the CNO on an annual basis. Work in partnership with the regional team to reach goals, meet targets, and deadlines for the mutual benefit and improvement of the quality of the facility and company.
    • Proficient and able to promote and teach the INTERACT standards.
    • Participate in monthly operations meetings as scheduled.

    • Director of Nursing Experience
    • Operational experience in Long Term Care and Skilled Nursing Setting
    • Regional consultant experience, preferred
    • Graduate of a state-approved school of nursing as a registered nurse
    • Current registration in good standing as a registered nurse in states assigned facilities are located, or ability and willingness to obtain
    • Knowledge of state and federal long-term care regulations
    • Excellent written and oral communication skills
    • Ability to interact and communicate with others effectively and professionally
    • Possess strong leadership and mentoring skills
    • Computer skills necessary to complete tasks as assigned.
    • Reliable transportation as travel is required to various facilities, conferences, and meetings and may include overnight stays.
