    Elderwood at Liverpool (formerly Birchwood) is searching for a energetic, bubbly & compassionate full time Certified Nursing Assistant Activity Leader to aide our very busy 160-bed Skilled Nursing facility in Liverpool, NY. We are undergoing current construction to update our facility including investing in new state of the art equipment for our activities staff! An active social life is an essential aspect of life at Elderwood. Our activities leaders conduct enjoyable recreation programs that keep residents energized, engaged and mentally stimulated.


    Certified Nursing Assistant/Memory Care Program Staff team members assist with ensuring the health and well-being of our residents by providing resident-related support services as well as therapeutic and social programming to meet the physical and emotional needs of residents

    CNA Activity Leader Duties:

    • Assists the Director Activities in planning and carrying out the activity program.

    • Approaches residents on assigned unit to plan individual recreational and social activities; reports to Director Activities about response and cooperation of resident.

    • Prepares resident information for use by the Director Activities in care planning and documentation; writes progress notes as assigned.

    • Assists in developing and maintaining good contact with community resources for the betterment of the activities program.

    • Responsible for maintaining departmental inventory of equipment and supplies.

    • Maintains attendance records and prepares other reports as required.

    • Maintains bulletin board displays on the nursing unit and in the Activities Room area.

    • Prepares & communicates daily broadcasting script.


    CNA Activity Leader Responsibilities:

    • Eighteen (18) years of age required.

    • High School diploma or equivalent preferred.

    • Registration in the New York State RHCF Nurse Aide Registry required.  Active on the New York State RHCF Nursing Aide Registry with no findings or convictions of resident abuse, mistreatment and misappropriation of property.

    • Experience with dementia population desired.

    • Ability to verbally communicate well with residents; families or agents; team mates and other departments required.

    • Ability to read and understand task assignments, and to report verbally and record care in writing required.

    • Display of genuine concern for the elderly; a dependable, patient and compassionate attitude towards residents required.

    • Demeanor always respectful of residents' rights and wishes required.

    • Ability to cooperate with others and to follow directions required.

    • Physical stamina for constant activity required.

    • Ability to perform tasks to established standards of excellence required.
