Lead a Rehab Program Where Your Clinical Judgment Still Matters

Elderwood at Hamburg is seeking a licensed Physical Therapist or Occupational Therapist to lead rehabilitation services within our 166-bed skilled nursing and rehabilitation community.

This is a hands-on leadership opportunity for an experienced therapy professional who wants to influence clinical quality, team performance, resident outcomes and the continued strength of a comprehensive rehabilitation program—without stepping away from patient care entirely.

As Director of Rehabilitation Services, you will lead physical, occupational and speech therapy operations while maintaining a clinical caseload at a minimum of 35% productivity. You will have visibility across the entire resident journey, from evaluation and care planning through treatment, progress, reimbursement and discharge.

You will work directly with the Administrator and collaborate with nursing, finance, medical records and other facility leaders. Your decisions will influence resident outcomes, employee development, regulatory performance, reimbursement, customer experience and the long-term success of rehabilitation services at Elderwood at Hamburg.

Responsibilities

What You’ll Lead as Director of Rehabilitation Services:

  • Direct the daily operations and performance of the rehabilitation department

  • Lead, orient, train and supervise therapists, therapy assistants, rehabilitation aides and assigned students

  • Oversee resident evaluations, treatment plans, progress notes, care plans and discharge summaries

  • Maintain a clinical caseload at a minimum of 35% productivity, preserving a direct connection to residents and clinical practice

  • Partner with nursing, medical providers and other interdisciplinary team members to integrate rehabilitation goals into each resident’s plan of care

  • Participate in resident and family care-plan meetings and communicate clearly about treatment goals, progress and expected outcomes

  • Monitor clinical quality, documentation, customer service, productivity and fiscal performance—and implement action plans when results require improvement

  • Oversee therapy-related MDS documentation, Medicare and insurance forms, billing information and restorative-program communication

  • Lead departmental meetings and contribute to quality assurance, safety, education and facility leadership initiatives

  • Develop the department’s annual budget and manage staffing, equipment, supplies and services within approved parameters

  • Review and update therapy policies and procedures to maintain compliance with federal and state requirements and professional practice standards

  • Provide education and in-service training to strengthen rehabilitation knowledge across the facility

Qualifications

What You’ll Bring as Director of Rehabilitation Services:

Required

  • Current New York State license as a Physical Therapist or Occupational Therapist

  • Professional education through an accredited PT or OT curriculum, including a bachelor’s degree

  • At least two years of supervisory or management experience

  • Current CPR/BLS certification

  • Ability to organize department operations, establish priorities, delegate effectively and promote teamwork

  • Strong written and verbal communication, decision-making and problem-solving skills

  • Commitment to resident rights, compassionate care and service excellence

Preferred

  • Occupational or physical therapy experience in geriatrics, skilled nursing, long-term care or post-acute rehabilitation

  • Experience with Microsoft Office and therapy-related clinical or documentation systems
