Mattapan Health & Rehabilitation Center is an 85-bed skilled nursing and rehabilitation facility in Mattapan, Massachusetts, providing Short-Term Rehabilitation and Long-Term Care services to the Greater Boston community.

We are seeking an experienced, mission-driven Licensed Nursing Home Administrator (LNHA) to lead the overall operations of Mattapan Health & Rehabilitation Center. This is an opportunity for a visible, people-centered healthcare leader who can balance high standards with genuine humanity—strengthening operations, developing leaders, supporting employees, championing quality, and remaining connected to the residents and families we serve.

The Leadership Opportunity

As Administrator, you will set the tone for Mattapan Health & Rehabilitation Center and provide leadership across every aspect of facility operations. You will work closely with department leaders, physicians, consultants, employees, residents, families, referral partners, and organizational leadership to ensure Mattapan remains a safe, responsive, well-managed, and welcoming care environment. Licensed Nursing Home Administrator (LNHA) -What You'll Lead:

Facility Operations & Compliance

• Oversee the day-to-day operations of the facility while ensuring compliance with Massachusetts and federal requirements
• Serve as the facility's designated Compliance Officer and ensure an effective, active compliance program
• Maintain readiness for regulatory surveys, inspections, audits, and accreditation requirements
• Lead corrective action and follow-up when compliance or operational concerns are identified
• Ensure infection control, safety, emergency preparedness, fire prevention, and disaster response programs remain current and effective
• Maintain and guide implementation of facility policies and procedures
• Monitor incident reports, complaints, and quality concerns and ensure appropriate investigation and follow-up
• Support a culture of Quality Assurance and Performance Improvement (QAPI) through data-driven decision-making and continuous improvement

Resident Care & Quality

• Ensure residents receive high-quality care and services that promote dignity, independence, safety, and quality of life
• Foster strong interdisciplinary collaboration across nursing, rehabilitation, social services, dietary, activities, medical, and support teams
• Ensure care planning, resident assessments, treatment, and documentation meet applicable standards
• Address resident, family, and responsible-party concerns promptly, respectfully, and effectively
• Promote resident rights, privacy, confidentiality, and freedom from abuse, neglect, exploitation, and discrimination
• Maintain a safe, clean, comfortable, and welcoming environment for residents, families, visitors, and employees
• Maintain visibility throughout the facility and develop meaningful relationships with residents, families, and staff

Leadership & Culture

• Lead, mentor, and hold accountable department heads and administrative leaders
• Establish clear operational, financial, quality, and programmatic goals for the facility
• Conduct regular leadership meetings to ensure communication, accountability, and operational alignment
• Recruit, develop, and retain qualified employees who support the facility's mission and standards
• Set the standard for integrity, professionalism, service excellence, and ethical conduct
• Encourage employee engagement, recognition, professional development, and continuous improvement
• Ensure required orientation, annual training, resident rights, safety, infection control, abuse prevention, and compliance education are completed
• Foster a culture where employees are encouraged to identify quality concerns and participate in meaningful solutions

Staffing, HR & Payroll Oversight

• Oversee staffing levels and ensure the facility has qualified personnel necessary to provide safe, effective resident care
• Provide leadership in the hiring, development, performance management, and retention of department heads and supervisory personnel
• Partner with Human Resources regarding employee relations, grievances, performance concerns, Workers' Compensation, FMLA, leave administration, and other personnel matters
• Maintain oversight of payroll accuracy, timekeeping, PTO, benefits inquiries, and related administrative processes
• Review and approve payroll documentation and ensure timely submission of required information
• Support ongoing employee education, training, and professional development
• Promote a respectful, accountable, and collaborative workplace culture

Financial & Census Accountability

• Participate in the development and management of the annual facility operating budget
• Monitor monthly financial performance and take action to address variances and control operating expenses
• Maintain strong facility census and support appropriate admissions and occupancy strategies
• Partner with admissions, sales, and marketing teams to support census growth and community awareness
• Monitor reimbursement-related processes and ensure required documentation is accurate and timely
• Oversee purchasing, requisitions, supplies, equipment, and contracted services necessary for facility operations
• Support capital planning and make recommendations regarding equipment, repairs, renovations, and facility improvements
• Maintain appropriate oversight of resident funds and financial transactions conducted at the facility

Community Presence & Reputation

• Serve as a primary representative of Mattapan Health & Rehabilitation Center to residents, families, referral partners, physicians, regulators, and the broader community
• Build and maintain strong relationships with hospitals, healthcare providers, community organizations, and professional partners
• Support marketing and outreach strategies that strengthen census, referrals, and the facility's reputation
• Promote positive relationships with the Long-Term Care Ombudsman and community partners
• Represent the facility with professionalism, transparency, and integrity
• Participate in community and professional organizations that support the facility and the residents it serves

 

Licensed Nursing Home Administrator (LNHA) -What We're Looking For:

• Bachelor's degree in Healthcare Administration, Business Administration, Nursing, or a related field required
• Advanced degree preferred
• Active Massachusetts Nursing Home Administrator (NHA) license required
• Minimum of three (3) years of experience as a Nursing Home Administrator in a skilled nursing, long-term care, or sub-acute care setting preferred
• Demonstrated experience leading facility operations, department heads, and interdisciplinary teams
• Strong knowledge of Massachusetts and federal regulations governing skilled nursing and long-term care facilities
• Demonstrated experience with regulatory compliance, quality improvement, budgeting, staffing, census management, and financial oversight
• Strong understanding of resident rights, HIPAA, OSHA, infection prevention, and healthcare compliance requirements
• Excellent verbal and written communication skills
• Strong problem-solving, decision-making, organizational, and interpersonal abilities
• A leadership style that is visible, ethical, calm under pressure, accountable, and people-centered
• Genuine commitment to providing exceptional care and service to older adults and individuals receiving skilled nursing and rehabilitation services
• This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit effective communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
