About the Position
As a Speech Language Pathologist at Elderwood at Ticonderoga, you will provide comprehensive speech-language therapy services to residents on our Rehabilitation Unit, helping individuals work toward improved communication, cognition, and swallowing function.
You will work closely with residents, families, nursing staff, and other members of the interdisciplinary team to develop individualized treatment plans and support positive outcomes.
What You'll Do
- Evaluate residents to identify speech, language, cognitive-communication, and swallowing needs.
- Develop and implement individualized treatment plans based on evaluation findings.
- Provide speech-language therapy in accordance with physician orders and facility standards.
- Monitor resident progress and adjust treatment plans as appropriate.
- Conduct audiometric screenings upon request and recommend additional audiology services when appropriate.
- Complete timely and accurate documentation, including evaluations, treatment plans, progress notes, and discharge summaries.
- Collaborate with nursing, therapy, and other interdisciplinary team members to coordinate resident care.
- Maintain compliance with federal, state, and facility regulations and standards.
What We're Looking For
- Master's degree in Speech-Language Pathology.
- Current New York State licensure as a Speech-Language Pathologist.
- Certificate of Clinical Competence (CCC-SLP) preferred.
- Experience in skilled nursing, rehabilitation, long-term care, or a related healthcare setting preferred.
- Strong communication, clinical, organizational, and documentation skills.
- A compassionate, resident-focused approach to care.
- Commitment to working collaboratively as part of an interdisciplinary team.
