Director of Social Services Overview

As the Director of Social Services, you'll oversee the daily operations of the Social Services Department while ensuring residents and their families receive compassionate guidance from admission through discharge.

Working closely with nursing leadership, physicians, therapists, and interdisciplinary team members, you'll advocate for residents, supervise social services staff, and help create individualized care plans that support each resident's physical, emotional, and psychosocial well-being.

 

This leadership role is ideal for someone who enjoys mentoring others while making a meaningful impact on the lives of older adults.

What You'll Do

  • Lead and oversee all Social Services Department operations.
  • Direct the admissions process and help ensure a smooth, welcoming transition for new residents and their families.
  • Coordinate discharge planning and facilitate safe, successful transitions to the next level of care.
  • Serve as a resident advocate while providing guidance and emotional support to residents and their loved ones.
  • Supervise, mentor, and evaluate Social Services team members while fostering a collaborative and supportive work environment.
  • Participate in interdisciplinary care planning meetings and help develop individualized resident care plans.
  • Coordinate PASARR Level II Annual Reviews and complete required SCREEN documentation for resident discharges.
  • Ensure timely, accurate documentation and compliance with all federal, state, and facility regulations.
  • Collaborate with the Social Work Consultant, Ombudsman, and community agencies to enhance resident services.
  • Educate staff on resident rights, abuse prevention, and the role of Social Services within the facility.
  • Identify opportunities to strengthen resident satisfaction, quality outcomes, and departmental performance.

Responsibilities

Responsibilities

  • Assumes responsibility for admission process for new residents as described in Section B of the Social Services Department Manual.
  • Serves as the Unit Social Worker for designated residents as described in Section B of the Social Services Department Manual, or delegates this responsibility according to current staffing pattern.
  • Serves as the Discharge Planner of the facility as described in Section B of the Social Services Manual; completes SCREEN upon discharge of resident.
  • Coordinates PASARR Level II Annual Review of residents.
  • Implements and oversees the policies and procedures of the Social Services Department in the areas of admission, social work, transfer and discharge services.
  • Participates in development of a comprehensive, inter-disciplinary health care and discharge plan for residents; attends Resident Care Planning and Resident/ Family Care Plan Review meetings and informs other disciplines of residents or family members who plan to attend.
  • Provides support and assistance to the Unit Social Worker in work with residents and families.
  • Supervises and evaluates the performances of the social services staff.
  • Works with the Social Work Consultant in matters of social services intervention or consultation.
  • Ensures all necessary documentation required for the Social Services Department.
  • Interprets to the professional staff of the facility the purpose and objectives of the Social Services Department.
  • Explains the Bill of Rights for Residents and Resident Abuse Reporting at General Orientation meetings, as scheduled.
  • Coordinates facility cooperation with the Ombudsman.

 

Qualifications

Qualifications

  • Minimum of a bachelor’s degree in social work from an accredited School of Social Work required.
  • One year of supervised social work experience in a health care setting working directly with individuals and one year of supervisory capacity preferred.
  • Experience in casework required.
  • Prior work with elders in a community or long-term care setting is preferred.
  • Must have skills in communication, assessment and social work methods and techniques.
  • Have a working understanding of social systems along with the ability to implement pertinent state and federal regulations.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
