About Elderwood at Burlington

Elderwood at Burlington is a 150-bed skilled nursing and subacute rehabilitation facility located in Burlington, Vermont. We provide comprehensive skilled nursing care, long-term care, and rehabilitation services including orthopedic rehabilitation, physical therapy, and speech therapy. Specialty programs include palliative care, respite care, and wound care, allowing us to support a wide range of resident needs with compassion and clinical excellence.

Position Overview

As a Social Worker, you will support residents and their families by assessing psychosocial, emotional, and environmental needs and connecting them with appropriate services and resources. You will serve as an advocate and key member of the interdisciplinary care team, helping ensure each resident’s care plan supports dignity, independence, and overall well-being.

This role involves meaningful engagement with residents throughout their entire care journey—from admission through discharge—ensuring they feel supported, heard, and empowered.

Key Responsibilities

  • Welcome and support residents and families during the admission process
  • Assess psychosocial, emotional, and support needs of residents
  • Participate in care planning and interdisciplinary team meetings
  • Assist with discharge planning and transitions of care
  • Complete required documentation, including admission and discharge forms (including SCREEN when applicable)
  • Provide counseling, emotional support, and resource referrals to residents and families
  • Advocate for resident rights, dignity, and quality of life
  • Collaborate with nursing, therapy, and clinical teams to support resident needs
  • Ensure compliance with facility policies and regulatory requirements
  • Participate in departmental meetings, training, and in-service programs

Responsibilities

Key Responsibilities

  • Welcome and assist new residents during the admission process
  • Assess residents' psychosocial, emotional, and support needs
  • Participate in the development and review of individualized care plans
  • Complete required admission and discharge documentation, including SCREEN forms when applicable
  • Assist with discharge planning and transitions to other levels of care
  • Document resident information according to facility policies and regulatory requirements
  • Provide counseling and support to residents and family members
  • Coordinate with interdisciplinary team members to address resident needs
  • Advocate for resident rights, dignity, and quality of life
  • Participate in departmental meetings and in-service training programs

Qualifications

  • Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human rehabilitation counseling, and psychology from an accredited institution; and services/ field including, but not limited to, sociology, gerontology, special education.
  • One year of supervised social work experience in a health care setting working directly with individuals
  • Experience in casework required.
  • Valid Drivers License and clean driving record required.
  • Work experience in the admission or discharge area of social work very desirable.
  • Desire and ability to work with the elderly and to cooperate with other staff members necessary.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
