Admissions Nurse – Position Overview:

As an Admissions Nurse, you will be responsible for screening prospective residents for placement within the Elderwood organization. You’ll be the vital first point of contact—helping families and referral sources navigate care options while ensuring each prospective resident is appropriately evaluated for services.

You will bring a well-rounded understanding of Elderwood’s services, staffing, routines, and clinical operations. Your role is key in supporting census growth, timely admissions, and a smooth transition process for new residents.

Key Responsibilities:

  • Conduct clinical screening of prospective residents

  • Communicate with referral sources, families, and facility teams

  • Collaborate with interdisciplinary teams to coordinate admissions

  • Maintain accurate and timely documentation

  • Represent Elderwood’s values and services in a professional, compassionate manner

Qualifications:

  • Active RN license in Vermont (or eligible for reciprocity)

  • Previous clinical experience in long-term care or admissions preferred

  • Strong communication and organizational skills

  • Comfort with working independently and collaboratively

Responsibilities

  • Assume responsibility for admission screenings and document compilation at local hospitals, ACF's, resident homes

  • Submit a statistical monthly report to the Director of Admissions indicating delineation of screens completed by facility referral, admissions placed and a narrative summary of key events.

  • Effectively utilize voice mail and two way pagers for the purpose of communication between facilities, screener and Director of Admissions.

  • Visit designated hospital assignments daily.

  • Maximize productivity through communication with Director of Admissions for re-assignment or assignment of secondary support.

  • Participate in targeted marketing projects.

  • Initiate physician, hospital and referral source marketing efforts.

  • Coordinate Pre-Admission Planning programs.

  • Coordinate with referral sources a timely transfer of appropriate residents to ElderWood sites.

 

Qualifications

  • Minimum 18 yrs. age required, High School diploma or equivalent required Baccalaureate Degree in Social Work; R.N.; or health related field from an accredited institution required. 2-3 years clinical experience, required.

  • Long-Term Care, Hospital Discharge Planning experience required. Strong clinical assessment skills and working knowledge of medical charts and terminology necessary. Ability to apply critical thinking skills.

  • Ability to meet the public easily, handle problems diplomatically, maintain flexibility and cooperate with other staff members necessary. Valid State & Driver’s License and clean driving record required with personal mode of transportation Must be energetic, enthusiastic and portray a positive attitude.

  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
