RN UAS Assessment Nurse: Full-Time Special Project (3 Months)

Elderwood IPA is seeking experienced Registered Nurses (RNs) to support a special community health assessment initiative.
In this role, the RN Assessment Nurse conducts New York State Uniform Assessment System (UAS) community health assessments for members in their homes or virtually. These assessments help determine functional status, care needs, and eligibility for long-term services.
This position is ideal for RNs who enjoy assessment-focused work, independent field-based practice, and supporting complex populations through thoughtful evaluation and documentation. Unlike bedside roles, this position focuses on comprehensive patient assessments, clinical evaluation, and care coordination insight that directly supports care management teams and member service planning.
Registered Nurses with experience in community health, long-term care, or MLTC assessment work are encouraged to apply and support this important community health initiative with Elderwood IPA.
Perform NYS UAS assessments in members’ homes or through virtual visits
Evaluate functional status, medical needs, behavioral health factors, and community supports
Complete required 6-month face-to-face reassessments
Identify changes in member condition, functional ability, and care needs
Compare reassessment findings to baseline data to monitor health status and risk factors
Recognize potential safety concerns or health deterioration and escalate appropriately
Communicate findings with care management teams, primary care providers, and interdisciplinary partners
Provide education to members and caregivers regarding chronic conditions, medications, and care planning
Complete detailed documentation in the electronic medical record and NYS UAS systems
Ensure assessments meet DOH, Medicaid, CMS, and regulatory requirements
Maintain compliance with HIPAA and Elderwood IPA policies
RN UAS Assessment Nurse:
Current Registered Nurse (RN) license in New York State in good standing
BSN, AAS, or Diploma in Nursing
Minimum three (3) years of nursing experience
At least one year of experience working with frail or elderly populations
Experience completing NYS UAS assessments strongly preferred
Valid New York State driver’s license
Comfortable conducting in-home and virtual assessments
Experience using EMR/EHR systems
