Community Health Worker - CNA Career Advancement Opportunity (Full-Time Project)

Support Patients Beyond the Bedside

 

Ideal for experienced CNAs or direct care professionals ready to expand into community health and care coordination.

Full-time hours required for approximately three months with the opportunity to transition to the Elderwood Network at project completion.

As a Community Health Worker, you will support individuals with complex health needs by connecting them to resources, identifying barriers to care, and helping ensure they receive the services they need to remain safe and stable in their homes and communities.

 

This is a non-clinical, patient-facing role that combines community visits, care coordination support, and quality initiatives to improve outcomes for vulnerable populations.

Special Project Opportunity

  • This role supports up to a 3-month full-time community health project through Elderwood IPA.
  • After the project period, employees may transition into per diem CNA opportunities through Elderwood Staffing Solutions, Elderwood’s internal float pool serving facilities throughout Western New York, with the ability to pick up flexible hours ranging from occasional shifts to part-time or full-time schedules. This is not required. 
  • This opportunity allows CNAs to gain community health experience while maintaining ongoing employment opportunities within the Elderwood organization.

Responsibilities

What You’ll Do as a CNA Community Health Worker:

Support Patients in the Community

  • Conduct in-home and community visits to check on patient safety, stability, and overall well-being
  • Identify changes in a patient’s condition, environment, or support system
  • Help patients access community resources, social services, and support programs
  • Encourage patient engagement, independence, and appropriate use of healthcare services

Act as a Patient Advocate

  • Build trusting relationships with patients and families
  • Communicate concerns or changes in patient needs to Care Managers and healthcare teams
  • Help identify barriers related to housing, food access, transportation, or social support

Support Care Coordination & Quality Initiatives

  • Assist with documentation review and care coordination support
  • Help identify gaps in care and follow-up needs
  • Participate in quality improvement activities and operational workflows that improve patient services

Documentation & Professional Practice

  • Maintain accurate documentation in the electronic medical record
  • Follow HIPAA and healthcare compliance standards
  • Represent the organization professionally during home and community visits

 

Qualifications

CNA Community Health Worker:

 

  • Associate degree in Health, Human Services, Social Work, or related field or equivalent healthcare experience
  • Minimum two years of experience supporting patients or vulnerable populations
  • CNA, patient care, home care, or community health experience strongly valued
  • Experience working with Medicare, Medicaid, or complex healthcare populations preferred
  • Comfortable traveling for home and community visits
  • Reliable transportation required
  • Bilingual (English/Spanish or other languages) preferred
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
