As a Program Specialist, you will play a critical role in shaping each child’s care journey - coordinating services, building individualized plans, and ensuring every child has the opportunity to reach their fullest potential.

Develop, manage, and update Individual Support Plans (ISP/IHP) and 6-month reviews
Coordinate interdisciplinary input to ensure comprehensive, person-centered care planning
Lead and facilitate required meetings in compliance with PA 3800/6400 and DHS regulations
Train and support staff on individualized plans, behavioral supports, and safety protocols
Monitor and document monthly and quarterly progress toward outcomes
Ensure all required services are delivered and aligned with each child’s goals
Maintain consistent communication with families, schools, and external providers
Participate in interdisciplinary, QA, and safety meetings
Support participation in community-based activities and life experiences
What You Bring as Program Specialist
Bachelor’s degree in Psychology, Sociology, Human Services, or related field + 2 years IDD experience
OR
Associate degree (60+ credits) in related field + 4 years IDD experience
Experience working directly with individuals with Intellectual and Developmental Disabilities (IDD)
Strong organizational, documentation, and coordination skills
Excellent communication and relationship-building ability
Comfort working across multiple disciplines and developmental levels
Proficiency with Microsoft Office and general computer systems
Valid driver’s license; ability to attend off-site meetings
CPR/First Aid Certification (or willingness to obtain)
