Elderwood at Hornell is seeking a compassionate and creative Activity Leader to join our team. In this role, you will help provide a dynamic and engaging social environment for residents—an essential part of maintaining quality of life and promoting emotional and cognitive well-being.
As an Activity Leader, you will plan, lead, and support recreational programs that keep residents energized, stimulated, and connected.

About Elderwood at Hornell
Elderwood at Hornell is a 112-bed Skilled Nursing Facility situated in New York’s Southern Tier. We provide comprehensive, personalized care including orthopedic, cardiac, and post-surgical rehabilitation, along with physical, occupational, and speech therapy. Additional services include Respite Care, Seasonal Memory Care, Adult Medical Day Services, and Palliative Care.

Assist the Director of Activities in planning and implementing group and individual activity programs.
Engage residents on assigned units to schedule and participate in recreational and social activities; report observations to the Director.
Prepare resident-specific information used in care planning; write progress notes as assigned.
Help maintain strong relationships with community resources to enhance activity offerings.
Maintain department equipment, supplies, and inventory.
Maintain attendance logs and prepare required documentation and reports.
Minimum 18 years of age; high school diploma or equivalent required.
Valid New York State Driver’s License with a clean driving record required.
Ability to develop strong working relationships with residents, staff, families, volunteers, and visitors required.
Ability to motivate and inspire residents and adapt activities to diverse needs required.
Understanding of resident rights and individual needs required.
Strong physical stamina to support frequent activity and movement required.
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
