LPN Per Diem Rate: $33.60/hour

As a Licensed Practical Nurse (LPN) at Elderwood, you will serve as a key clinical team leader responsible for delivering high-quality, person-centered care in a supportive, home-like setting. You will oversee daily clinical operations, collaborate with an interdisciplinary team, and ensure that care is delivered according to regulatory standards and resident needs.

Administer medications and treatments as ordered by the physician, utilizing the five rights of medication administration
Plan, implement, and evaluate resident care
Direct and monitor the personal care duties and nursing care procedures carried out by Nursing Assistants
Assist Nursing Assistants with direct care of residents, as needed
Report changes in resident condition, unusual symptoms, daily needs, and progress to the Unit Manager, Assistant Unit Manager, or Charge Nurse Supervisor
Follow each resident’s Plan of Care and ensure Nursing Assistants are performing care according to the plan
Receive, store, and properly manage controlled substances and other medications; ensure safe placement and use of medications in carts, cupboards, refrigerators, and narcotic cabinets
Document in Nursing Notes, Medication Administration Records, Treatment Records, care plans, and all other required records
Promote a safe, respectful, and resident-centered care environment
LPN - Licensed Practical Nurse Skills/Competencies:
Required:
Active LPN license in the state of Vermont (or eligibility for reciprocity)
Strong communication skills, both verbal and written
Exceptional customer service and problem-solving abilities
Ability to supervise and work effectively with other personnel
Caring and respectful attitude toward residents
Physical stamina to meet the demands of the position
Proficient computer skills
Preferred: Training or work experience in a geriatric or long-term care setting
