Elderwood at Lockport is a 126-bed skilled nursing facility in a park-like setting near the historic Erie Canal. We specialize in Rehabilitation Services and Memory Care, offering meaningful opportunities for clinical professionals who want to make a true impact.

Pay Ranges (Based on title, experience, and applicable programs)

  • PT / OT / SLP: $33.00 – $44.86/hr
  • COTA: $22.00 – $31.00/hr
  • CNA: $17.50 – $24.41/hr
  • LPN: $33.66 – $38.42/hr
  • RN: $34.19 – $53.00/hr

Responsibilities

LPN - Licensed Practical Nurse:

  • Licensed Practical Nurses (LPNs) are indispensable members of our interdisciplinary team and are charged with managing the daily clinical regimen of each resident and sub-acute patient. Our LPNs enjoy a unique opportunity to gain extensive clinical experience and provide advanced care in a home-like setting.

RN - Registered Nurse:

  • RN Team Leaders assist in ensuring the general health and well-being of our residents by providing direct nursing care. Direct nursing care of residents encompasses planning, developing, organizing, implementing assessing, and evaluating the care of residents to ensure that the highest degree of quality care and regulatory compliance is maintained at all times.

CNA - Certified Nursing Assistant

  • Certified Nursing Assistants (CNA) provide personal and nursing care to meet the physical and emotional needs of residents in Elderwood skilled nursing facilities (SNF). They are under the direction of a licensed nurse. State specific certification is required.

PT - Physical Therapist:

  • Physical Therapy team members assist with ensuring the health and well-being of our residents by providing comprehensive and quality therapy services. This position is responsible for administering skilled physical therapy treatments ensuring that the highest degree of quality resident care is maintained at all times.

SLP - Speech-Language Therapist:

  • Speech-Language Therapy team members assist with ensuring the health and well-being of our residents by making recommendations for a speech-language therapy treatment plan, if necessary, and carrying out treatments as planned; conducting an audiometric screening of residents upon request and making recommendations for further audiology service.

OT - Occupational Therapist:

  • Occupational Therapy team members assist with ensuring the health and well-being of our residents by providing comprehensive and quality therapy services. This position is responsible for administering skilled occupational therapy treatments ensuring that the highest degree of quality resident care is maintained at all times.

 

COTA - Certified Occupational Therapy Assistant:

  • COTA team members assist with ensuring the health and well-being of our residents by being responsible for carrying out occupational health treatments as developed and recommended by a Licensed Occupational Therapist.

RT - Respiratory Therapist:

  • Respiratory Therapist team members assist with ensuring the health and well-being of our residents by carrying out skilled respiratory therapy treatments.

Qualifications

(LPN - Licensed Practical Nurse):

  • Credentials as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current state license required.

  • LPN Training and work experience in a geriatric setting preferred.

  • Ability to supervise and work well with other personnel required.

  • Ability to communicate well verbally and in writing required.

  • Caring, respectful attitude towards residents required.

  • Physical stamina for constant activity required.

  • Ability to perform tasks to established standards of excellence required.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
