Elderwood at Waverly is seeking a dedicated, compassionate, and collaborative Adult Day Services Manager to join our team. In this Monday-Friday 8am-4pm position, we are seeking an RN to oversee our Adult Medical Day Services program.

 

Adult Day Services Manager - Position Overview:

 

The Adult Medical Day Services Manager is responsible for providing social services, organizing services for registrants, oversight of day-to-day management, administration of services, and assigning adequate and appropriate licensed personnel to always be on duty.

Elderwood at Waverly is a 200-bed skilled nursing facility (SNF) located in the valley of Tioga County. Our beautifully renovated facility boasts updated technology, convenient parking for staff members, and employee appreciation initiatives throughout the year. Our services and programs include Oncology, Orthopedic Rehab, SMARTS Express Care, and Wound Care, as well as Specialty Programs including Adult Medical Day Services, Outpatient Therapy, Palliative Care, Respite Care and Seasons Memory Care.

Responsibilities

  • Ensures that psycho-social needs are assessed, evaluated and recorded, and that services are provided to meet the identified needs as part of the coordinated care plan.
  • Ensures the following program components are available:
    • Case management, including health education
    • Interdisciplinary care planning
    • Nursing services
    • Nutrition
    • Social services
    • Assistance with activities of daily living
    • Planned therapeutic or recreational activities
    • Pharmaceutical services
  • Provides social services ensuring registrant psycho-social needs are assessed, evaluated, and recorded, and that services are provided to meet the identified needs as part of the coordinated care plan.
  • Ensures arrangements are made for the use of and/or access to other community resources as needed.
  • Ensures a comprehensive assessment is completed upon admission and every six months thereafter per regulation.
  • Ensures a clinical record for each registrant is kept confidential, intact, and readily available
  • Supervise direct care staff ensuring registrants receive assistance in accordance with their plan of care.
  • Create a Monthly Activity Calendar
  • Manage staff to address ADLs and programming needs and documentation throughout the day.
  • Manages and completes documentation of daily programs, nursing clinical needs, appointments, and ADLs daily.
  • Monthly audits performed on documentation and assessments. These audits are submitted to the administrator.
  • Weekly and Monthly reports are submitted to the administrator based on the following information:
    • Nursing Clinical Needs
    • Social leisure Program participation
    • ADLs
    • Medical Appointments
    • Scheduling of upcoming appointments
    • Assessment and documentation completion

Qualifications

  • Graduate of an accredited nursing school. 
  • Current active nursing license required (RN). 
  • At least one year of supervisory experience required.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
