Outreach Specialist Overview

 

The Outreach Specialist promotes Elderwood IPA, drives awareness of partnering relationships and identifies membership growth opportunities in the WNY territory. The Outreach Specialist will be able to educate individuals on Managed Long Term Care benefits. The Outreach Specialist will also build and establish relationships within the community to generate new enrollment opportunities with individuals, groups, providers, community-based organizations and local business leaders. The Outreach Specialist serves as an Elderwood IPA advocate at Community and Networking Events and acts as a liaison in the Community, as well as with Providers.

Responsibilities

Outreach Specialist Overview

  • Provide external outreach on behalf of Elderwood IPA to establish and maintain key relationships within designated territory to generate potential enrollment opportunities for high quality referrals from relevant organizations
  • Outreach to a diverse and vulnerable population and travel to urban, suburban and rural areas in 7 counties in WNY (approximately 70% out of office, primarily Erie / Niagara, Monroe/ Chautauqua, some Genesee, infrequent Orleans/ Wyoming)
  • Attend networking or other events
  • Regular follow-up with potential contacts and established relationships to discuss services provided by Elderwood IPA and partnering organizations
  • Keep list updated based on change of contacts- introduction to new contacts
  • Coordinate with some provider outreach teams (fiscal intermediaries) with recurring meetings to share related events and other outreach activities
  • Update and review partners list tracker with Director to identify partners to contact in next month and quarter and return on effort for highest potential lead sources
  • Interact with interested potential enrollees to answer product and/or service questions at communities
  • Education on Elderwood IPA’s mission, partnering organization services, MLTC model of care, services provided and basic enrollment qualifications
  • Assess needs with basic screen of candidates to identify qualified, eligible potentials related to both long-term care needs and Medicaid coverage for enrollment
  • Address questions and concerns, effectively and clearly present benefits of products in accordance with all Governmental regulations
  • Complete pre-enrollment screening form for all potential enrollees
  • Provide linkage for potential enrollee information to partnering organization for further follow up and qualification
  • Complete all necessary paperwork, documentation, and follow-up as required (i.e. lead reports, outreach logs, scheduling and other administrative work).

Qualifications

 Educational Requirements and Qualifications

  • Valid NYS Driver’s License
  • Bachelor’s Degree in Social Work or related field and 3-5 years of related experience OR Associates Degree with 7 years of related experience
  • Bilingual fluency in Spanish, highly preferred
  • Proficiency in Microsoft Office: Excel, Word, Access; Internet navigation, required

  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
